Forms Requiring Parental Action
Non-Health Services forms will be available electronically via the parent portal, at hillsideschool.myschoolapp.com, where you can electronically sign and submit.
The Health Services Forms need to be downloaded via the link below, filled out, and uploaded via the SNAP system. You will receive instructions from the nurse's office regarding this in August.
